Objective: To analyze the temporal trend and the association between overweight and female breast cancer mortality in Brazil. Methods: Ecological time-series study that investigated data on overweight from the Surveillance of Risk and Protective Factors for Chronic Diseases by Telephone Survey (VIGITEL), and information on deaths from malignant neoplasm of the breast obtained from the Online Mortality Atlas. The period of analysis comprised the years 2006 to 2018, including the 26 state capitals and the Federal District, grouped into their geographic regions of the country. The percentage variation over the period and the biennial percentage changes were calculated using Prais–Winsten regression. Results: Overweight and obesity showed an increasing trend between 2006 and 2018 in all regions of Brazil, with the greatest increase observed in the North (percentage variation over the period [PVP]=51.6%; p=0.001) and Southeast (PVP=97.2%; p=0.001). At the national level, both elevated body weight status (variation parameter [VP]=1.34; p=0.017) and obesity (VP=2.27; p=0.041) showed a positive association with the increasing trend in breast cancer mortality. Conclusion: The findings indicate that the increase in adiposity in the country is related to a worsening of breast cancer mortality indicators, reinforcing the need for prevention strategies and weight control as a public health priority in order to mitigate this impact in the Brazilian context.
