Pharmaceutical care for patients with hypertension: a new methodology and the importance of this practice in home care

Objective: To evaluate and increase the knowledge of patients about hypertension trying toguide and motivate them for the correct use of medication and for the adherence to adequate health styles, pointing out the importance of pharmaceutical home care in the treatment of the illness. Methods: A prospective, experimental and longitudinal study held with 10 randomly selected patients from a municipal pharmacy. Data on life style, information concerning the used drugs and results after the accomplishment of the study were collected by the application of initial and final forms. The patients’ pressure values in the first and last weeks were analyzed, using the Student’s t-test (bilateral) considering ?= 0.05 and ANOVA (Only Factor). Results: The reduction of 9.89 % and 8.57 % in the values of systolic blood pressure (SBP) and Diastolic blood pressure (DBP), respectively, were observed, occurring greater reduction of blood pressure (BP) between the second and fourth weeks. The levels of BP obtained smaller standard deviation in the second month, suggesting greater stability of these after the accomplishment of the study. Errors related to the medicines were corrected, achieving a gradual reduction of blood pressure of 7 patients, without causing changes in pharmacotherapy. The patients referred to feel safe, educated and motivated, being this important for a greater adherence to medication. Conclusion: The study suggests that pharmaceutical home care is important for monitoring the hypertension patient, since it decreases the variations of blood pressure, important factor for the patient’s safety.Downloads
