				<title>RESUMO</title>
				<sec>
					<title>Objetivo:</title>
					<p> Analisar as evidências científicas acerca da importância da Educação Permanente em Saúde como estratégia para a prevenção da violência institucional dentro da Rede de Atenção à Saúde. </p>
				</sec>
				<sec>
					<title>Método:</title>
					<p> Revisão integrativa seguindo as diretrizes <italic>Preferred Reporting Items for Systematic Reviews and Meta-Analyses</italic>. A busca foi realizada, em setembro de 2024, utilizando os seguintes descritores: Pessoal de Saúde; Educação Continuada; e Violência no Trabalho. Inicialmente, foram identificados 37 estudos, dos quais 7 foram selecionados para análise após a aplicação dos critérios. </p>
				</sec>
				<sec>
					<title>Resultados:</title>
					<p> Foram identificadas quatro categorias: a I abordou as formas de violência institucional, como negligência, discriminação e abuso de poder, evidenciando os principais fatores que contribuem para sua ocorrência; a II discutiu a implementação da Educação Permanente, destacando sua importância para capacitar os profissionais de saúde, melhorar a comunicação interprofissional e promover uma cultura de segurança; a III envolveu as práticas de acolhimento e humanização, mostrando como a formação contínua pode ajudar a prevenir e reduzir casos de violência institucional; e a IV abordou os desafios institucionais e condições de trabalho no sistema de saúde. </p>
				</sec>
				<sec>
					<title>Conclusão:</title>
					<p> A Educação Permanente se configura como uma estratégia essencial na prevenção da violência institucional na Rede de Atenção à Saúde. Contudo, sua eficácia depende de um compromisso institucional contínuo com a formação dos profissionais, da implementação de políticas de segurança e da promoção de uma cultura organizacional de respeito e cuidado. O fortalecimento das práticas educacionais voltadas para a humanização e o acolhimento são relevantes para garantir um ambiente seguro, ético e de qualidade, tanto para os usuários quanto para os profissionais de saúde.</p>

					<title><italic>Objetivo:</italic></title>
					<p><italic>Analizar la evidencia científica sobre la importancia de la Educación Permanente en Salud como estrategia para la prevención de la violencia institucional dentro de la Red de Atención a la Salud.</italic></p>

					<title><italic>Método:</italic></title>
					<p><italic>Revisión integrativa siguiendo las directrices Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). La búsqueda se realizó en septiembre de 2024 utilizando los siguientes descriptores: Personal de Salud; Educación Continua; y Violencia en el Trabajo. Inicialmente se identificaron 37 estudios, de los cuales 7 fueron seleccionados para análisis tras la aplicación de los criterios establecidos.</italic></p>

					<title><italic>Resultados:</italic></title>
					<p><italic>Se identificaron cuatro categorías: la primera abordó las formas de violencia institucional, tales como negligencia, discriminación y abuso de poder, evidenciando los principales factores que contribuyen a su ocurrencia; la segunda discutió la implementación de la Educación Permanente, destacando su importancia para capacitar a los profesionales de la salud, mejorar la comunicación interprofesional y promover una cultura de seguridad; la tercera se centró en las prácticas de acogida y humanización, mostrando cómo la formación continua puede contribuir a prevenir y reducir casos de violencia institucional; y la cuarta abordó los desafíos institucionales y las condiciones laborales en el sistema de salud.</italic></p>

				<sec>
					<title><italic>Conclusión:</italic></title>
					<p><italic>La Educación Permanente se configura como una estrategia esencial para la prevención de la violencia institucional en la Red de Atención a la Salud. No obstante, su eficacia depende de un compromiso institucional continuo con la formación de los profesionales, de la implementación de políticas de seguridad y de la promoción de una cultura organizacional basada en el respeto y el cuidado. El fortalecimiento de las prácticas educativas orientadas a la humanización y al acogimiento resulta fundamental para garantizar un entorno seguro, ético y de calidad, tanto para los usuarios como para los profesionales de la salud.</italic></p>

			<title>INTRODUÇÃO</title>
			<p>A Rede de Atenção à Saúde (RAS), implementada pela Portaria nº 4.279, de 30 de dezembro de 2010, representa a principal estratégia do Sistema Único de Saúde (SUS) para garantir o cuidado integral aos usuários, além de possibilitar a otimização de recursos e a maior eficiência do sistema. A Atenção Primária à Saúde (APS) é um dos componentes essenciais da RAS, desempenhando um papel central no processo de cuidado, à medida que constitui o ponto de contato inicial entre os usuários e os profissionais de saúde. A RAS abrange um conjunto de ações e serviços voltados à prevenção, promoção e reabilitação da saúde, de forma individual e coletiva. Além disso, atua como um centro de comunicação dentro do sistema, o que lhe confere grande potencial para o desenvolvimento de ações e estratégias voltadas ao enfrentamento da violência, incluindo aquela perpetrada por profissionais de saúde contra os usuários<xref ref-type="bibr" rid="B1"><sup>1</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B2"><sup>2</sup></xref><sup>).</sup></p>
			<p>O acolhimento, no contexto das equipes da Rede de Atenção à Saúde, em especial pelas equipes da Atenção Básica, é uma estratégia eficaz para identificar e minimizar os danos e sofrimento de pessoas em situação de violência, sem potencializá-los<xref ref-type="bibr" rid="B1"><sup>1</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B3"><sup>3</sup></xref>. A violência, entendida como um fenômeno social, refere-se a comportamentos de indivíduos, grupos, classes ou povos, que resulta em danos materiais e/ou psicológicos, como agressão física, violência psicológica e violência institucional, afetando tanto a vítima quanto o agressor<xref ref-type="bibr" rid="B4"><sup>4</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B5"><sup>5</sup></xref>. Nesse contexto, a violência afeta a população de maneira desigual, sendo evidenciada diante das diversas vulnerabilidades presentes na sociedade. O sistema de saúde é um espaço fundamental para abordar este tema, e os profissionais possuem a responsabilidade de contribuir para a prevenção, atender aos casos e desenvolver ações efetivas no enfrentamento da violência<xref ref-type="bibr" rid="B6"><sup>6</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B7"><sup>7</sup></xref>.</p>
			<p>Ademais, a violência institucional é compreendida como uma questão de saúde não pontual, pois sua constância e disseminação estão enraizadas na relação entre os serviços e os usuários frequentemente, resultando em ações violentas. Esta violência geralmente está expressa em situações de negligência na assistência, em todas as formas de discriminação social, violência física e até sexual<xref ref-type="bibr" rid="B8"><sup>8</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B9"><sup>9</sup></xref>. Uma das estratégias para trabalhar esse problema é a Educação Permanente em Saúde (EPS), uma abordagem de ensino e aprendizagem que visa melhorar a qualidade do trabalho. A EPS promove a construção coletiva de conhecimentos, com base nas necessidades dos profissionais, valorizando suas experiências. Além disso, deve-se entender como um processo político-pedagógico, permitindo que os envolvidos se comprometam em construir conhecimentos e relações com a comunidade para transformar realidades e promover a saúde, numa perspectiva freiriana<xref ref-type="bibr" rid="B10"><sup>10</sup></xref>.</p>
			<p>A EPS pode ser realizada em qualquer espaço que favoreça seus efeitos e promova a interação entre os profissionais, no qual a colaboração se torna essencial para garantir a qualidade dos serviços, atendendo de forma eficaz às necessidades dos usuários. Apresenta o objetivo de progredir por meio de um ensino problematizador e crítico do cotidiano, pela produção de conhecimento no dia a dia do trabalho, pela criação de novas práticas assistenciais, soluções criativas e iniciativas que transformam a realidade do cuidado, transformando, consequentemente, os modos de produzi-lo<xref ref-type="bibr" rid="B10"><sup>10</sup></xref><sup>), (</sup><xref ref-type="bibr" rid="B11"><sup>11</sup></xref>.</p>
			<p>Nesse sentido, o objetivo deste estudo foi analisar as evidências científicas acerca da importância da Educação Permanente como estratégia para a prevenção da violência institucional dentro da Rede de Atenção à Saúde.</p>
		</sec>
		<sec>
			<title>MÉTODO</title>
			<p>Trata-se de uma revisão integrativa da literatura, cujo objetivo é sintetizar - de forma sistemática, organizada e abrangente - os achados de pesquisas relacionadas ao tema em questão. Esse tipo de revisão possibilita uma compreensão mais ampla e aprofundada do assunto investigado, sendo considerado um método de pesquisa essencial no contexto da Prática Baseada em Evidências (PBE), por favorecer a incorporação de evidências na prática clínica. Sua finalidade é reunir e consolidar os resultados de estudos sobre uma temática ou questão específica, proporcionando uma análise detalhada, crítica e estruturada da produção científica disponível<xref ref-type="bibr" rid="B12"><sup>12</sup></xref>.</p>
			<p>Como base no presente estudo, foram utilizadas seis etapas para a conclusão desta revisão: 1) Elaboração de pergunta norteadora; 2) Busca ou amostragem na literatura; 3) Coleta de dados; 4) Análise crítica dos estudos incluídos; 5) Discussão dos resultados; 6) Apresentação da revisão integrativa<xref ref-type="bibr" rid="B13"><sup>13</sup></xref>. A revisão do processo foi baseada nas recomendações da lista de conferência <italic>Preferred Reporting Items for Systematic Reviews and Meta-Analyses</italic> (PRISMA).</p>
			<p>Para elaboração da questão de pesquisa foi utilizada a estratégia PICo (P: população; I: Intervenção; Co: Outcome (desfecho ou resultado esperado). Esse acrônimo pode ser utilizado para construção de questões variadas, possibilitando a definição correta das informações necessárias para a resolutividade da questão de pesquisa. Além disso, maximiza a recuperação de evidências nas diversas bases de dados, foca o escopo da pesquisa e evita a realização de buscas desnecessárias<xref ref-type="bibr" rid="B14"><sup>14</sup></xref>.</p>

			<p>Com base na <xref ref-type="table" rid="t1">Tabela 1</xref>, foi elaborada a seguinte questão: Como a Educação Permanente pode contribuir para a prevenção da violência institucional na Rede de Atenção à Saúde?</p>
			<p>A busca dos dados foi desenvolvida no mês setembro de 2024 no qual, a partir de uma seleção de artigos com acesso on-line nas bases de dados eletrônicas <italic>Medical Literature Analysis and Retrieval System Online</italic> (MEDLINE), por meio da <italic>National Library of Medicine National Institutes of Health</italic> (PUBMED); Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Base de Dados em Enfermagem (BDENF) via Biblioteca Virtual em Saúde (BVS). Para a construção da estratégia de busca, utilizaram-se descritores não controlados e controlados registrados no Descritores em Ciências da Saúde (DeCS) e no Medical Subject Headings (MeSH): P: Pessoal de Saúde; I: Educação Continuada; Co: Violência no Trabalho. Os operadores booleanos AND e OR foram utilizados para realizar os cruzamentos entre os descritores. A estratégia de busca pode ser delimitada como uma ferramenta, ou grupo de regras, para tornar admissível o confronto entre uma pergunta gerada e a informação registrada em uma base de dados.</p>

			<p>A seleção dos estudos elegíveis foi realizada por meio da leitura dos títulos e resumos de cada trabalho, aplicando-se critérios de inclusão e exclusão para classificá-los como aceitos ou rejeitados. Incluíram-se estudos publicados e disponíveis integralmente e com acesso livre nas bases de dados buscadas; documentos do tipo artigo de estudo primário; em inglês, português ou espanhol; que abordassem conjuntamente os seguintes aspectos: atenção primária à saúde, violência no trabalho e/ou institucional e Educação Permanente.</p>
			<p>A busca nas bases de dados, identificaram-se 37 publicações, sendo: zero publicações na MEDLINE, 27 na LILACS e 10 na BDENF. Após leitura dos títulos e resumos, obteve-se um total de 16 estudos. Após a avaliação dos critérios de exclusão, foram selecionados sete artigos que compõem a amostra final do respectivo estudo de revisão, conforme mostra o fluxograma abaixo.</p>

			<p>Foram excluídos: os trabalhos duplicados ou que tratavam de outra temática, como violência obstétrica. A seleção foi realizada por dois dos autores e, em seguida, conferida por um terceiro avaliador, sendo as divergências de classificação resolvidas por consenso. Os dados de cada artigo foram extraídos por dois revisores, de forma independente, sendo identificadas informações sobre: autor, ano, local de estudo, idioma, objetivo, desenho do estudo, nível de evidência, participantes, categoria temática do estudo, principais achados.</p>
			<p>O nível de evidência foi baseado na classificação apresentada no estudo de Souza, Silva e Carvalho<xref ref-type="bibr" rid="B13"><sup>13</sup></xref>: Nível 1 - evidências resultantes da meta-análise de múltiplos estudos clínicos controlados e randomizados; Nível 2 - evidências obtidas em estudos individuais com delineamento experimental; Nível 3 - evidências de estudos quase-experimentais; Nível 4 - evidências de estudos descritivos (não experimentais) ou com abordagem qualitativa; Nível 5- evidências provenientes de relatos de caso ou de experiência; Nível 6 - evidências baseadas em opiniões de especialistas.</p>
			<p>Após a extração dos dados, foi realizada a análise de conteúdo, conforme proposta por Bardin<xref ref-type="bibr" rid="B15"><sup>15</sup></xref>, uma análise qualitativa temática dos achados dos estudos incluídos. Essa análise consistiu na leitura exaustiva dos textos completos, buscando identificar elementos comuns, divergências e padrões recorrentes relacionados à Educação Permanente e à prevenção da violência institucional. Os conteúdos foram organizados em categorias temáticas construídas a partir da recorrência dos principais aspectos abordados nos estudos. A categorização foi realizada de forma independente por dois pesquisadores e, em caso de discordância, foi alcançado consenso por meio de discussão entre os revisores.</p>
		</sec>
		<sec sec-type="results|discussion">
			<title>RESULTADOS E DISCUSSÃO</title>
			<p>Dos 19 estudos elegíveis, 12 foram excluídos após a leitura do texto na íntegra. Desse modo, a amostra desta revisão integrativa totalizou sete artigos. A síntese dos artigos selecionados está apresentada no <xref ref-type="table" rid="t3">Quadro 1</xref> e <xref ref-type="table" rid="t4">Quadro 2</xref>.</p>

			<p>Ao analisar o ano de publicação dos estudos incluídos na revisão integrativa, observa-se uma maior concentração de estudos recentes, com 28,6% publicados em 2023 e 28,6% em 2022. Apenas 14,3% dos estudos foram publicados em 2024 e outros 28,6% datam de 2020. Essa distribuição indica que as questões abordadas, como violência institucional e práticas de cuidado, permanecem atuais e relevantes no contexto da saúde, sendo frequentemente investigadas nos últimos anos. A base de dados comum a todos os estudos foi a LILACS (100%), mostrando sua relevância na indexação de pesquisas relacionadas à saúde pública e ao contexto latino-americano. O fato de todos os estudos serem provenientes de uma única base de dados pode ser interpretado como uma limitação em termos de abrangência.</p>
			<p>A totalidade dos estudos analisados utilizou abordagens qualitativas, destacando-se como método predominante para compreender fenômenos complexos, como violência institucional e equidade no sistema de saúde. Dentro dessa abordagem, diferentes estratégias metodológicas foram aplicadas, incluindo entrevistas semiestruturadas (28,6%), grupos focais (28,6%) e perspectivas teóricas específicas, como a triangulação teórica (14,3%) e o Paradigma da Complexidade (14,3%). O <xref ref-type="table" rid="t4">Quadro 2</xref> apresenta as características dos estudos, como local e idioma, nível de evidência, tipo de violência mais evidenciadas e a importância da Educação Permanente frente à violência institucional identificadas nos estudos.</p>

			<p>Relacionado ao idioma, 100% dos estudos foram publicados em português, o que facilita a acessibilidade para profissionais e pesquisadores nacionais, mas pode restringir a disseminação internacional e a ampliação do impacto científico global. Todos os estudos apresentaram nível de evidência 4 (100%), o que denota uma concentração em estudos descritivos e qualitativos. Embora isso possa indicar uma fragilidade metodológica, ressalta-se que tais estudos são fundamentais para compreender fenômenos complexos e multifacetados, como a violência institucional, especialmente quando envolvem aspectos subjetivos, culturais e éticos do cuidado em saúde.</p>
			<p>Todos os estudos foram realizados no Brasil, com distribuição concentrada em algumas regiões. Essa predominância pode estar relacionada à escolha dos descritores e à própria formulação da pergunta de pesquisa, voltada à realidade da Rede de Atenção à Saúde no contexto brasileiro, especialmente no âmbito do Sistema Único de Saúde (SUS). A região Sul e a região Nordeste contribuíram com 28,6% dos estudos cada, enquanto a região Sudeste também representou 28,6%. Apenas 14,3% dos estudos abordaram todas as cinco regiões do Brasil, conforme exemplificado no <xref ref-type="table" rid="t4">Quadro 2</xref>. O Estado com mais estudos foi Santa Catarina com 60%. Essa distribuição evidencia uma sub-representação das regiões Norte e Centro-Oeste, sugerindo que a produção científica ainda não cobre integralmente as especificidades e desigualdades regionais no sistema de saúde brasileiro. </p>
			<p>Os artigos selecionados foram distribuídos em quatro categorias temáticas a partir da técnica de análise proposta por Bardin<xref ref-type="bibr" rid="B15"><sup>15</sup></xref>: Fragmentação do Cuidado e Impacto da Violência Institucional; Desafios no Reconhecimento da Violência e Atendimento Humanizado; Papel Transformador da Educação Permanente para a Qualidade do Cuidado; Desafios Institucionais e Condições de Trabalho no Sistema de Saúde.</p>
			<sec>
				<title>Fragmentação do Cuidado e Impacto da Violência Institucional</title>
				<p>A fragmentação do cuidado no sistema de saúde brasileiro e sua relação com a violência institucional são questões centrais nos estudos analisados. Uma das principais causas dessa fragmentação é a falta de articulação efetiva entre os diferentes serviços e níveis de atendimento, o que compromete a eficácia do cuidado e perpetua a violência institucional. A fragilidade da rede intersetorial impede que os casos de violência sejam abordados de forma integral e contínua, resultando em atendimentos desarticulados e sem resolutividade<xref ref-type="bibr" rid="B21"><sup>21</sup></xref>. A ausência de vínculos institucionais fortes e de coordenação entre os serviços aumenta a vulnerabilidade dos usuários, que acabam sendo “jogados” de um serviço para outro, sem um acompanhamento adequado<xref ref-type="bibr" rid="B20"><sup>20</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>.</p>
				<p>Por outro lado, embora a fragmentação seja uma característica estrutural do sistema, ela é também influenciada pela falta de processos de comunicação entre os profissionais e as instituições. Muitos profissionais de saúde não estão plenamente cientes das redes de apoio disponíveis e, muitas vezes, o encaminhamento para outros serviços é realizado de forma improvisada e sem a devida continuidade. Isso contribui para o agravamento da violência institucional, onde os usuários enfrentam barreiras para o acesso a cuidados adequados e a serviços especializados<xref ref-type="bibr" rid="B17"><sup>17</sup></xref>. A abordagem intersetorial poderia ser uma solução viável para superar a fragmentação, mas essa abordagem depende diretamente da formação contínua dos profissionais, de forma que eles compreendem a importância de trabalhar de maneira integrada com outros serviços e setores, como educação, assistência social e segurança pública<xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>).</sup></p>
				<p>A Educação Permanente é, portanto, uma ferramenta essencial para tentar reduzir a fragmentação do cuidado. A capacitação constante dos profissionais de saúde, com foco na integração entre os serviços e na compreensão das diversas dimensões da violência institucional, pode melhorar significativamente a articulação entre as equipes. No entanto, como apontam os estudos, essa formação precisa ser contínua e adaptada às realidades locais, considerando as especificidades de cada região e comunidade<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>. Ainda, a falta de uma implementação eficaz da Educação Permanente contribui para a perpetuação da fragmentação e da violência institucional, reforçando a necessidade de políticas públicas que garantam a formação e a infraestrutura necessárias para a prática integrada.</p>
				<p>Essa realidade se contrapõe ao princípio da integralidade do cuidado, conforme proposto por Barbara Starfield<xref ref-type="bibr" rid="B23"><sup>23</sup></xref>, que defende a atenção primária como coordenadora do cuidado e promotora de continuidade e longitudinalidade. A fragmentação identificada também revela a ausência de práticas pautadas na promoção da saúde, que, conforme a Política Nacional de Promoção da Saúde<xref ref-type="bibr" rid="B24"><sup>24</sup></xref>, exige articulação intersetorial e a criação de ambientes saudáveis e acolhedores. A desarticulação entre os serviços e a lógica biomédica ainda predominante limitam a efetividade das ações voltadas para populações vulnerabilizadas<xref ref-type="bibr" rid="B25"><sup>25</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B26"><sup>26</sup></xref>. Assim, além da formação profissional, é necessário um esforço sistêmico de reorganização dos serviços a partir dos princípios do SUS, em especial a integralidade, a equidade e a participação social.</p>
				<p>Neste contexto, a Educação Permanente se configura como ferramenta estratégica para a promoção da saúde, ao capacitar os profissionais para práticas integradas, contextualizadas e centradas nas necessidades reais dos usuários. A formação contínua, adaptada às especificidades locais, contribui para a superação da fragmentação e para a construção de uma rede de cuidado mais coesa e efetiva, alinhada aos princípios do Sistema Único de Saúde (SUS).</p>
			</sec>
			<sec>
				<title>Desafios no Reconhecimento da Violência e Atendimento Humanizado</title>
				<p>A falta de reconhecimento da violência, especialmente em suas formas mais sutis, como a psicológica e institucional, é um problema recorrente nos estudos analisados. Muitos profissionais de saúde não reconhecem as especificidades das populações vulneráveis, como as pessoas trans, e, frequentemente, minimizam as violências sofridas por essas pessoas, negando-lhes um atendimento adequado<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>. Nesse sentido, a marginalização de casais homoafetivos e as dificuldades desses usuários em acessar um cuidado que reconheça suas necessidades específicas representam desafios importantes para o sistema de saúde. As barreiras culturais e a falta de capacitação dos profissionais para lidar com essa diversidade de demandas perpetuam a violência institucional. O atendimento é frequentemente pautado por um olhar normativo e excludente, onde as necessidades do usuário são negligenciadas em favor de um modelo biomédico que não contempla as especificidades de cada indivíduo<xref ref-type="bibr" rid="B16"><sup>16</sup></xref>. </p>
				<p>De forma contrária, em contextos de alta vulnerabilidade social, os profissionais de saúde podem até reconhecer a violência, mas carecem de estratégias eficazes para lidar com essas situações. Desse modo, embora a violência seja identificada, a escassez de recursos e a falta de suporte institucional tornam a intervenção limitada, levando a um atendimento reativo e não preventivo<xref ref-type="bibr" rid="B20"><sup>20</sup></xref>. Nesse sentido, embora as formações acadêmicas avancem em relação à violência, muitos profissionais ainda se sentem desconfortáveis ao lidar com casos complexos de violência, especialmente quando não envolvem agressões físicas visíveis, como os casos de violência psicológica ou institucional<xref ref-type="bibr" rid="B19"><sup>19</sup></xref>.</p>
				<p>A Educação Permanente surge como a resposta necessária para superar essa lacuna no reconhecimento da violência. A capacitação dos profissionais de saúde deve envolver não apenas a atualização técnica, mas também o desenvolvimento de uma compreensão crítica sobre as dinâmicas sociais e culturais que influenciam as experiências de violência. Essa formação deve ser abrangente, abordando questões de gênero, orientação sexual, etnia e classe social<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>.</p>
				<p>A dificuldade de reconhecimento da violência institucional também está ligada a uma compreensão limitada do conceito de saúde, ainda centrada na ausência de doença. É necessário, então, superar o modelo centrado na clínica médica e avançar para uma abordagem que considere os sujeitos em sua integralidade, reconhecendo as dimensões simbólicas, culturais e subjetivas da produção do cuidado<xref ref-type="bibr" rid="B27"><sup>27</sup></xref>. Essa perspectiva é reforçada por Ayres<xref ref-type="bibr" rid="B26"><sup>26</sup></xref>, ao propor o conceito de “cuidado ampliado”, que implica reconhecer os usuários como sujeitos de direito, e não apenas como portadores de sintomas. A promoção da saúde, nesse sentido, requer que os serviços estejam atentos às formas sutis de violência - como a negligência, o julgamento moral e a omissão -, adotando uma postura ética e responsiva às necessidades diversas dos usuários.</p>
			</sec>
			<sec>
				<title>Papel Transformador da Educação Permanente para a Qualidade do Cuidado</title>
				<p>Os estudos concordam que a Educação Permanente tem um papel transformador na melhoria da qualidade do cuidado e no enfrentamento da violência institucional. Quando a Educação Permanente é integrada com a articulação intersetorial, ela não apenas melhora a resolutividade do atendimento, mas também promove uma compreensão mais ampla e integral das necessidades dos usuários<xref ref-type="bibr" rid="B21"><sup>21</sup></xref>. Já a formação contínua é crucial para superar o modelo biomédico, permitindo que os profissionais adotem práticas de cuidado mais inclusivas e baseadas nas necessidades reais dos usuários<xref ref-type="bibr" rid="B19"><sup>19</sup></xref>. </p>
				<p>Porém, embora a Educação Permanente tenha grande potencial transformador, sua implementação é limitada pela falta de suporte institucional e pela escassez de recursos. Mesmo com a capacitação contínua, os profissionais de saúde frequentemente se deparam com a falta de infraestrutura, recursos materiais e apoio institucional, o que compromete a eficácia das intervenções<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>. Nesse sentido, existe ainda uma falta de alinhamento entre os programas de Educação Permanente e as necessidades específicas das comunidades atendidas<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>. Isso significa que, muitas vezes, as formações não estão suficientemente contextualizadas para lidar com as realidades locais, limitando seu impacto no cotidiano dos serviços de saúde.</p>
				<p>Assim, embora a Educação Permanente seja reconhecida como um pilar fundamental para a melhoria do cuidado, sua implementação depende de um esforço coordenado que vá além da capacitação técnica. Ela precisa ser acompanhada de políticas públicas que garantam a infraestrutura necessária, como a contratação de mais profissionais, a criação de protocolos de atendimento intersetoriais e a melhoria das condições de trabalho nas unidades de saúde. Além disso, a Educação Permanente deve ser adaptada às realidades locais, considerando as particularidades culturais, sociais e econômicas de cada região, para que possa ser efetivamente aplicada no atendimento diário<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>.</p>
				<p>A articulação intersetorial e o trabalho em rede são fundamentais para enfrentar as múltiplas dimensões da violência institucional<xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B21"><sup>21</sup></xref>. Defende-se que a violência não pode ser tratada de forma isolada por um único serviço, sendo necessária a colaboração entre diferentes setores - como saúde, educação, segurança pública e assistência social - para garantir uma resposta eficaz. No entanto, a falta de integração entre esses setores é um dos maiores obstáculos para um atendimento eficaz. Os profissionais de saúde frequentemente desconhecem os recursos disponíveis em outras áreas e têm dificuldade em fazer os encaminhamentos necessários para garantir a continuidade do cuidado. Essa falta de articulação resulta em falhas no atendimento e na exclusão de muitas pessoas dos serviços de saúde e proteção<xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B21"><sup>21</sup></xref>.</p>
				<p>Apesar de a articulação intersetorial ser uma diretriz importante no SUS, ela ainda enfrenta desafios significativos, como a falta de uma visão holística do processo de cuidado por parte dos profissionais de saúde e a escassez de recursos financeiros e humanos para implementar ações intersetoriais efetivas. A Educação Permanente tem o potencial de superar essas barreiras, capacitando os profissionais para reconhecerem a importância do trabalho em rede e a colaboração com outros serviços. No entanto, para que isso se concretize, é necessário que haja uma mudança cultural nas instituições de saúde, que deve ser promovida<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
				<p>A Educação Permanente, como política estruturante no SUS, é um instrumento central para operacionalizar a Promoção da Saúde no cotidiano dos serviços. A EPS deve provocar mudanças nas práticas a partir da problematização do processo de trabalho, integrando saberes técnicos e populares<xref ref-type="bibr" rid="B29"><sup>29</sup></xref>. No entanto, como já apontado, há um descompasso entre o que propõe é proposto pela política e o que se efetiva nos territórios. A EPS precisa estar comprometida com a transformação das práticas e não apenas com a atualização técnica. Isso implica reconhecer que a promoção da saúde não é uma ação pontual, mas um modo de organizar o cuidado e o processo de trabalho<xref ref-type="bibr" rid="B30"><sup>30</sup></xref>. A construção de espaços de aprendizagem crítica nos serviços pode ser o caminho para incorporar as dimensões éticas, culturais e subjetivas que permeiam a violência institucional.</p>
			</sec>
			<sec>
				<title>Desafios Institucionais e Condições de Trabalho no Sistema de Saúde</title>
				<p>A relação significativa entre o aumento das cargas de trabalho dos gestores na Estratégia Saúde da Família (ESF) e os aspectos macropolíticos institucionais têm sido amplamente discutidos. A falta de autonomia e planejamento afeta significativamente os gestores, gerando estresse, frustração e ansiedade pela ausência de controle sobre os processos de trabalho e pelo apoio insuficiente da equipe. Devido a essa falta de autonomia, os gestores enfrentam dificuldades para implementar mudanças eficazes nos processos de trabalho, o que impacta negativamente a qualidade do atendimento<xref ref-type="bibr" rid="B19"><sup>19</sup></xref>.</p>
				<p>É comum que os profissionais excedam a carga horária diária, o que resulta em esgotamento físico e mental. As longas jornadas de trabalho, associadas às más condições laborais, geram uma grande demanda física e emocional, prejudicando o bem-estar dos profissionais de saúde e, consequentemente, afetando a qualidade do cuidado prestado. A pressão constante sobre os profissionais leva à insatisfação, ao aumento do estresse e à dificuldade de manutenção de um atendimento humanizado e eficaz<xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
				<p>Os fatores - como baixa remuneração, insatisfação e carga horária prolongada - têm um impacto significativo na saúde mental dos profissionais de saúde. Quanto mais exausto emocionalmente o trabalhador se sentia, mais insatisfeito se considerava, o que poderia configurar violência institucional<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>. Esses fatores revelam a relação entre a precariedade das condições de trabalho e a violência institucional, resultando em um ciclo de desgaste emocional e físico que afeta a qualidade do atendimento. </p>
				<p>Além disso, a falta de condições apropriadas de trabalho contribui para um cenário de estresse, tornando as instituições de saúde mais propensas à violência institucional estruturada. A violência se manifesta por ação ou omissão nas instituições de saúde, com a falta de acesso e a má qualidade dos serviços prestados. As regras e as relações burocráticas perpetuam desigualdades e uma assimetria de poder, contribuindo para a precariedade nas condições de trabalho e o baixo desempenho no atendimento, o que pode gerar violência institucional<xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
				<p>A precarização das condições de trabalho, associada à lógica produtivista imposta aos serviços de saúde, configura um obstáculo significativo à promoção da saúde e à superação da violência institucional. As relações de trabalho no SUS ainda são marcadas pela instabilidade, sobrecarga e baixa valorização, o que compromete a capacidade dos profissionais de atuarem de forma crítica e transformadora. A Promoção da Saúde, nesse contexto, exige ações dirigidas aos usuários, e estratégias de cuidado com os trabalhadores do SUS, por meio de ambientes organizacionais saudáveis, valorização profissional e escuta qualificada<xref ref-type="bibr" rid="B31"><sup>31</sup></xref>. O investimento em Educação Permanente precisa estar articulado a essas estratégias, favorecendo a criação de espaços de escuta, reflexão e mudança nas práticas institucionais.</p>
			</sec>
		</sec>
		<sec sec-type="conclusions">
			<title>CONSIDERAÇÕES FINAIS</title>
			<p>Este estudo teve como objetivo analisar as evidências científicas acerca da importância da Educação Permanente em Saúde como estratégia para a prevenção da violência institucional dentro da Rede de Atenção à Saúde. Os achados demonstram que a Educação Permanente pode desempenhar papel central na sensibilização dos profissionais, na qualificação do cuidado e na construção de práticas mais humanizadas e éticas nos serviços de saúde. A violência institucional - em suas múltiplas formas, como negligência, discriminação, abusos de poder e maus-tratos -, tem sido identificada como um obstáculo significativo para a efetividade dos serviços de saúde, impactando diretamente a experiência dos usuários e a moral dos profissionais de saúde. </p>
			<p>Apesar dos achados responderem à questão norteadora deste estudo, observou-se que todos os estudos incluídos são brasileiros e apresentam nível de evidência 4, o que evidencia uma limitação metodológica. Soma-se a isso a ausência de estudos com delineamentos mais robustos, como estudos de avaliações de impacto, e a exclusão da literatura cinzenta, que pode ter restringido o escopo das evidências identificadas. A categorização temática dos dados, embora realizada de forma criteriosa, também carrega o risco de subjetividade na análise. Destrate, mesmo com essas limitações, os resultados apontam que a Educação Permanente pode ser uma ferramenta pontual para transformar práticas institucionais violentas e desumanizadas. Seu uso, quando estruturado de forma interprofissional, crítica e continuada, tem o potencial de impactar positivamente a qualidade do cuidado em saúde, especialmente em serviços públicos onde se concentram as principais queixas relacionadas à violência institucional.</p>
			<p>Os achados evidenciam uma lacuna significativa na produção de conhecimento empírico sobre o tema, especialmente no que se refere à mensuração de resultados objetivos e sustentáveis das ações de EPS sobre a prevenção da violência institucional. Investir em pesquisas com abordagens qualitativas e quantitativas mais robustas pode fortalecer o campo e oferecer evidências mais sólidas para formulação de políticas públicas. </p>
			<p>Diante disso, pesquisas futuras são necessárias para aprofundar a compreensão sobre como a Educação Permanente pode ser adaptada a diferentes contextos, como os específicos de comunidades vulneráveis e populações marginalizadas, como a população LGBTQIA+, pessoas em situação de rua e outros grupos historicamente excluídos do acesso à saúde. É fundamental investigar também as condições de implementação da Educação Permanente, como a adaptação dos programas de formação às realidades locais e os desafios encontrados pelos profissionais na aplicação dos conhecimentos adquiridos. Além disso, estudos que avaliem a eficácia da Educação Permanente na redução concreta da violência institucional são essenciais para validar as intervenções e propor melhorias. O desenvolvimento de metodologias participativas, que envolvam os próprios usuários na construção do conhecimento e na formação dos profissionais, pode ser uma importante área a ser explorada. </p>

			<abstract>
				<title>ABSTRACT </title>
				<sec>
					<title>Objective: </title>
					<p>To analyze the scientific evidence regarding the importance of Continuing Health Education as a strategy for preventing institutional violence within the healthcare network. </p>
				</sec>
				<sec>
					<title>Method:</title>
					<p> This integrative review followed the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. The search was carried out in September 2024, using the following descriptors: “Health Personnel”, “Continuing Education”, “Violence at Work”. Initially, 37 studies were identified, of which 7 were selected for analysis after applying the criteria. Initially, 37 studies were identified. 				<sec>
					<title>Results:</title>
					<p> Four main categories were identified. 1ª: addressed forms of institutional violence such as neglect, discrimination, and abuse of power, highlighting the main factors contributing to their occurrence. 2ª: discussed the implementation of Continuing Education, emphasizing its importance in empowering healthcare professionals, improving interprofessional communication, and promoting a culture of safety. 3ª: involved practices of welcoming and humanization, demonstrating how ongoing training can help prevent and reduce cases of institutional violence. 4ª: addressed Institutional Challenges and Working Conditions within the Health System. </p>

				<sec>
					<title>Conclusion:</title>
					<p> Continuing Health Education emerges as an essential strategy for preventing institutional violence in the healthcare network. However, its effectiveness depends on a continuous institutional commitment to professional training, the implementation of safety policies, and the promotion of an organizational culture based on respect and care. Strengthening educational practices aimed at humanization and welcoming is crucial to ensuring a safe, ethical, and high-quality environment for both users and healthcare professionals.</p>

				<title>INTRODUCTION</title>
				<p>The Health Care Network (RAS), as established by Ordinance No. 4,279 of December 30, 2010, represents the primary strategy of the Unified Health System (SUS) to provide comprehensive care for users, while also improving resource use and system efficiency. Primary Health Care (PHC) is a key component of the RAS, playing a central role in the care process as it is the first point of contact between users and health professionals. The RAS includes a range of actions and services focused on prevention, health promotion, and rehabilitation, both individually and collectively. Additionally, it functions as a communication hub within the system, offering significant potential for developing actions and strategies to address violence, including that perpetrated by health professionals against users<xref ref-type="bibr" rid="B1"><sup>1</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B2"><sup>2</sup></xref>.</p>
				<p>Reception, within the context of Health Care Network teams, especially by Primary Care teams, is an effective strategy to identify and reduce harm and suffering for people in situations of violence, without making them worse<xref ref-type="bibr" rid="B1"><sup>1</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B3"><sup>3</sup></xref>. Violence, seen as a social phenomenon, refers to the actions of individuals, groups, classes, or peoples that cause material and/or psychological harm, such as physical assault, psychological violence, and institutional violence, impacting both the victim and the perpetrator<xref ref-type="bibr" rid="B4"><sup>4</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B5"><sup>5</sup></xref>. In this context, violence affects populations unequally, especially in the face of various vulnerabilities in society. The health system is a key space to address this issue, and professionals must contribute to prevention, respond to cases, and develop effective actions to fight violence<xref ref-type="bibr" rid="B6"><sup>6</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B7"><sup>7</sup></xref>.</p>
				<p>Furthermore, institutional violence is seen as a non-specific health issue because its persistence and spread are rooted in the relationship between services and users, often leading to violent actions. This violence typically manifests in neglectful care, social discrimination, physical violence, and even sexual violence<xref ref-type="bibr" rid="B8"><sup>8</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B9"><sup>9</sup></xref>. One way to address this problem is through Permanent Health Education (EPS), an approach to teaching and learning that aims to improve the quality of work. EPS encourages the collective building of knowledge based on the needs of professionals, valuing their experiences. Additionally, it should be viewed as a political-pedagogical process that enables participants to commit to creating knowledge and fostering relationships with the community to transform realities and promote health, following a Freirean perspective<xref ref-type="bibr" rid="B10"><sup>10</sup></xref>.</p>
				<p>EPS can be carried out in any space that favours its effects and promotes interaction among professionals, in which collaboration becomes essential to guarantee the quality of services, effectively meeting the needs of users. It presents the objective of progressing through problematizing and critically teaching of everyday life, through the production of knowledge in everyday work, through the creation of new care practices, creative solutions, and initiatives that transform the reality of care, consequently changing the ways of producing it<xref ref-type="bibr" rid="B10"><sup>10</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B11"><sup>11</sup></xref>.</p>
				<p>In this sense, the objective of this study was to analyze the scientific evidence on the importance of Permanent Education as a strategy for institutional violence prevention within the Health Care Network.</p>
			</sec>
			<sec sec-type="methods">
				<title>METHOD</title>
				<p>It is an integrative literature review, the objective of which is to synthesize, in a systematic, organized, and comprehensive manner, the findings of research related to the topic in question. This type of review enables a broader and more in-depth understanding of the subject studied, and it is considered an essential research method in the context of Evidence-Based Practice (EBP), as it favors the incorporation of evidence into clinical practice. Its purpose is to gather and consolidate the results of studies on a specific topic or issue, providing a detailed, critical, and structured analysis of the available scientific production <xref ref-type="bibr" rid="B12"><sup>12</sup></xref>.</p>
				<p>As a basis for this study, six steps were used to complete this review: 1) Development of guiding questions; 2) Search or sampling in literature; 3) Data collection; 4) Critical analysis of included studies; 5) Discussion of results; 6) Presentation of the integrative review<xref ref-type="bibr" rid="B13"><sup>13</sup></xref>. The review process was based on the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist.</p>
				<p>To develop the research question, the PICo strategy was used (P: population; I: Intervention; Co: Outcome (expected outcome or result). This acronym can be used to construct various questions, enabling the correct definition of the information necessary to resolve the research question. Furthermore, it maximizes the recovery of evidence in numerous databases, focuses the research scope, and avoids unnecessary searches<xref ref-type="bibr" rid="B14"><sup>14</sup></xref>.</p>

				<p>Based on <xref ref-type="table" rid="t5">Table 1</xref>, the following question was formulated: How can Continuing Education contribute to the prevention of institutional violence in the Health Care Network?</p>
				<p>The data search was developed in September 2024, based on a selection of articles with online access in the electronic databases Medical Literature Analysis and Retrieval System Online (MEDLINE), through the National Library of Medicine National Institutes of Health (PUBMED); Latin American and Caribbean Literature in Health Sciences (LILACS) and Nursing Database (BDENF) via the Virtual Health Library (VHL). To construct the search strategy, uncontrolled and controlled descriptors registered in the Health Sciences Descriptors (DeCS) and Medical Subject Headings (MeSH) were used: O: Health Personnel, E: Continuing Education, and C: Workplace Violence. The Boolean operators AND and OR were used to perform cross-referencing between the descriptors. The search strategy can be defined as a tool or a set of rules to enable the comparison between a generated query and the information recorded in a database.</p>

				<p>The selection of eligible studies was carried out by reading the titles and abstracts of each work, applying inclusion and exclusion criteria to classify them as accepted or rejected. Studies published and available in full and with free access in the databases searched were included; documents of the primary study article type; in English, Portuguese, or Spanish; that jointly addressed the following aspects: primary health care, violence in the workplace and/or in institutions, and Continuing Education.</p>
				<p> The search in the databases identified 37 publications, being: zero publications in MEDLINE, 27 in LILACS, and 10 in BDENF. After reading the titles and abstracts, a total of 16 studies were obtained. After evaluating the exclusion criteria, seven articles were selected to make up the final sample for the respective review study, as shown in the flowchart below.</p>

				<p>Duplicate papers or those dealing with other topics, such as obstetric violence, were excluded. Selection was made by two authors and then reviewed by a third reviewer; any disagreements in classification were resolved by consensus. Data from each article were extracted independently by two reviewers, identifying information on: author, year, study location, language, objective, study design, level of evidence, participants, thematic category of the study, and main findings.</p>
				<p>The level of evidence was based on the classification presented in the study by Souza, Silva and Carvalho<xref ref-type="bibr" rid="B13"><sup>13</sup></xref>: Level 1 - evidence resulting from the meta-analysis of multiple controlled and randomized clinical studies; Level 2 - evidence obtained in individual studies with experimental design; Level 3 - evidence from quasi-experimental studies; Level 4 - evidence from descriptive (non-experimental) studies or with a qualitative approach; Level 5 - evidence from case reports or experience; Level 6 - evidence based on expert opinions.</p>
				<p>After data extraction, content analysis was performed, as proposed by Bardin<xref ref-type="bibr" rid="B15"><sup>15</sup></xref>, a qualitative thematic analysis of the findings from the included studies. This analysis consisted of an exhaustive reading of the complete texts, seeking to identify common elements, divergences, and recurring patterns related to Permanent Education and the prevention of institutional violence. The content was organized into thematic categories based on the recurrence of the main aspects addressed in the studies. Categorization was performed independently by two researchers; in case of disagreement, consensus was reached through discussion among the reviewers.</p>
			</sec>
			<sec sec-type="results|discussion">
				<title>RESULTS AND DISCUSSION</title>
				<p>Of the 19 eligible studies, 12 were excluded after full-text reading. Thus, the sample for this integrative review totalled seven articles. A summary of the selected articles is presented in <xref ref-type="table" rid="t7">Charts 1</xref> and <xref ref-type="table" rid="t8">2</xref>.</p>

				<p>When analyzing the year of publication of the studies included in the integrative review, a greater concentration of recent studies is observed, with 28.6% published in 2023 and 28.6% in 2022. Only 14.3% of the studies were published in 2024, and another 28.6% date from 2020. This distribution indicates that the issues addressed, such as institutional violence and care practices, remain current and relevant in the health context, having been frequently investigated in recent years. The database common to all studies was LILACS (100%), demonstrating its relevance in indexing research related to public health and the Latin American context. The fact that all studies come from a single database can be interpreted as a limitation in terms of scope.</p>
				<p>All the studies analyzed used qualitative approaches, standing out as the predominant method for understanding complex phenomena, such as institutional violence and equity in the health system. Within this approach, different methodological strategies were applied, including semi-structured interviews (28.6%), focus groups (28.6%), and specific theoretical perspectives, such as theoretical triangulation (14.3%) and the Complexity Paradigm (14.3%). <xref ref-type="table" rid="t8">Table 2</xref> presents the characteristics of the studies, such as location and language, level of evidence, type of violence most highlighted, and the importance of Continuing Education in the face of institutional violence identified in the studies.</p>

				<p>Regarding language, 100% of the studies were published in Portuguese, which facilitates accessibility for national professionals and researchers but may restrict international dissemination and the expansion of global scientific impact. All studies presented level of evidence 4 (100%), which denotes a concentration in descriptive and qualitative papers. Although this may indicate a methodological weakness, it is pertinent to emphasize that such studies are fundamental to understanding complex and multifaceted phenomena, such as institutional violence, especially when they involve subjective, cultural, and ethical aspects of health care.</p>
				<p>All studies were conducted in Brazil, with distribution concentrated in a few regions. This predominance may be related to the choice of descriptors and the formulation of the research question itself, which focused on the reality of the Healthcare Network in the Brazilian context, especially within the Unified Health System (SUS). The South and Northeast regions each contributed 28.6% of the studies, while the Southeast region also represented 28.6%. Only 14.3% of the studies addressed all five areas of Brazil, as shown in <xref ref-type="table" rid="t8">Table 2</xref>. The state with the most studies was Santa Catarina, with 60%. This distribution highlights an underrepresentation of the North and Central-West regions, suggesting that scientific production still does not fully address the regional specificities and inequalities in the Brazilian healthcare system. </p>
				<p>The selected articles were distributed into four thematic categories based on the analysis technique proposed by Bardin<xref ref-type="bibr" rid="B15"><sup>15</sup></xref>: Fragmentation of Care and the Impact of Institutional Violence; Challenges in Recognizing Violence and Providing Humanized Care; The Transformative Role of Continuing Education for Quality of Care; and Institutional Challenges and Working Conditions in the Health System.</p>
				<sec>
					<title>Fragmentation of Care and the Impact of Institutional Violence</title>
					<p>The fragmentation of care in the Brazilian health system and its relationship with institutional violence are central issues in the studies analyzed. One of the primary causes of this fragmentation is the lack of effective coordination between different services and levels of care, which compromises the effectiveness of care and perpetuates institutional violence. The fragility of the intersectoral network prevents cases of violence from being addressed comprehensively and continuously, resulting in disjointed and ineffective care<xref ref-type="bibr" rid="B21"><sup>21</sup></xref>. The lack of strong institutional links and coordination between services increases the vulnerability of users, who end up being “thrown” from one service to another, without adequate monitoring<xref ref-type="bibr" rid="B20"><sup>20</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>.</p>
					<p>On the other hand, although fragmentation is a structural characteristic of the system, it is also influenced by the lack of communication processes between professionals and institutions. Many health professionals are not fully aware of the support networks available, and often, referrals to other services are made in an improvised manner and without continuity. This contributes to the worsening of institutional violence, where users face barriers to accessing adequate care and specialized services<xref ref-type="bibr" rid="B17"><sup>17</sup></xref>. An intersectoral approach could be a viable solution to overcome fragmentation, but this approach directly depends on the ongoing training of professionals, so that they understand the importance of working in an integrated manner with other services and sectors, such as education, social assistance, and public safety<xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>).</sup></p>
					<p>Continuing Education is, therefore, an essential tool in trying to reduce the fragmentation of care. Ongoing training of health professionals, focusing on integration among services and understanding the various dimensions of institutional violence, can significantly improve coordination between teams. However, as studies point out, this training needs to be continuous and adapted to local realities, considering the specificities of each region and community <xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>), (</sup><xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>), (</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>. Furthermore, the lack of effective implementation of Continuing Education contributes to the perpetuation of fragmentation and institutional violence, reinforcing the need for public policies that guarantee the training and infrastructure necessary for integrated practice.</p>
					<p>This reality contradicts the principle of comprehensive care, as proposed by Barbara Starfield<xref ref-type="bibr" rid="B23"><sup>23</sup></xref>, who defends primary care as a coordinator of care and promoter of continuity and longitudinality. The fragmentation identified also reveals the absence of practices based on health promotion, which, according to the National Health Promotion Policy<xref ref-type="bibr" rid="B24"><sup>24</sup></xref>, requires intersectoral coordination and the creation of healthy and welcoming environments. The disconnect between services and the still predominant biomedical logic limits the effectiveness of actions aimed at vulnerable populations<xref ref-type="bibr" rid="B25"><sup>25</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B26"><sup>26</sup></xref>. Therefore, in addition to professional training, a systemic effort is needed to reorganize services based on the principles of the SUS, especially comprehensiveness, equity, and social participation.</p>
					<p>In this context, Continuing Education is configured as a strategic tool for health promotion, by training professionals for integrated, contextualized practices focused on the real needs of users. Continuous training, adapted to local specificities, contributes to overcoming fragmentation and building a more cohesive and effective care network, aligned with the principles of the Unified Health System (SUS).</p>
				</sec>
				<sec>
					<title>Challenges in Recognizing Violence and Providing Humanized Care</title>
					<p>The lack of recognition of violence, especially in its subtler forms, such as psychological and institutional, is a recurring problem in the studies analyzed. Many health professionals fail to recognize the specific needs of vulnerable populations, such as trans people, and often minimize the violence these people suffer, denying them adequate care<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>. In this sense, the marginalization of same-sex couples and the difficulties these users have in accessing care that recognizes their specific needs represent significant challenges for the health system. Cultural barriers and the lack of training among professionals to deal with this diversity of demands perpetuate institutional violence. Care is often guided by a normative and exclusionary perspective, where the user’s needs are neglected in favor of a biomedical model that does not consider the specificities of each individual<xref ref-type="bibr" rid="B16"><sup>16</sup></xref>. </p>
					<p>Conversely, in contexts of high social vulnerability, health professionals may recognize violence but lack effective strategies to deal with these situations. Thus, although violence is identified, the scarcity of resources and the lack of institutional support make intervention limited, leading to reactive rather than preventive care<xref ref-type="bibr" rid="B20"><sup>20</sup></xref>. In this sense, although academic training advances concerning violence, many professionals still feel uncomfortable dealing with complex cases of violence, especially when they do not involve visible physical aggression, such as cases of psychological or institutional violence<xref ref-type="bibr" rid="B19"><sup>19</sup></xref>.</p>
					<p>Continuing Education emerges as a necessary response to overcome this gap in the recognition of violence. The training of health professionals must involve not only technical updating, but also the development of a critical understanding of the social and cultural dynamics that influence experiences of violence. This training must be comprehensive, addressing issues of gender, sexual orientation, ethnicity, and social class<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>.</p>
					<p>The difficulty in recognizing institutional violence is also linked to a limited understanding of the concept of health, still centered on the absence of disease. It is therefore necessary to overcome the model centered on clinical medicine and move towards an approach that considers subjects in their entirety, recognizing the symbolic, cultural, and subjective dimensions of care production<xref ref-type="bibr" rid="B27"><sup>27</sup></xref>. This perspective is reinforced by Ayres<xref ref-type="bibr" rid="B26"><sup>26</sup></xref> when proposing the concept of “extended care”, which implies recognizing users as subjects of rights, and not just as carriers of symptoms. Health promotion, in this sense, requires that services be attentive to subtle forms of violence - such as negligence, moral judgment, and omission -, adopting an ethical and responsive stance to the diverse needs of users.</p>
				</sec>
				<sec>
					<title>The Transformative Role of Continuing Education for Quality of Care</title>
					<p>Studies agree that Continuing Education has a transformative role in improving the quality of care and addressing institutional violence. When Continuing Education is integrated with intersectoral coordination, it not only improves the resolution of services but also promotes a broader and more comprehensive understanding of users’ needs<xref ref-type="bibr" rid="B21"><sup>21</sup></xref>. Continuing education is crucial for overcoming the biomedical model, allowing professionals to adopt more inclusive care practices based on the real needs of users<xref ref-type="bibr" rid="B19"><sup>19</sup></xref>. </p>
					<p>However, although Continuing Education has enormous transformative potential, its implementation is limited by the lack of institutional support and the scarcity of resources. Even with ongoing training, health professionals often face a lack of infrastructure, material resources, and institutional support, which compromises the effectiveness of interventions<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>. In this sense, there is still a lack of alignment between Continuing Education programs and the specific needs of the communities served<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>. It means that training is often not sufficiently contextualized to address local realities, limiting its impact on the day-to-day running of health services.</p>
					<p>Thus, although Continuing Education is recognized as a fundamental pillar for improving care, its implementation depends on a coordinated effort that goes beyond technical training. It needs to be accompanied by public policies that guarantee the necessary infrastructure, such as hiring more professionals, creating intersectoral care protocols, and improving working conditions in health units. Furthermore, Continuing Education must be adapted to local realities, considering the cultural, social, and economic particularities of each region, so that it can be effectively applied in daily care<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>.</p>
					<p>Intersectoral coordination and networking are essential for addressing the multiple dimensions of institutional violence<xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B21"><sup>21</sup></xref>. It is argued that violence cannot be treated in isolation by a single service, and that collaboration between different sectors - such as health, education, public safety, and social assistance - is necessary to ensure an effective response. However, the lack of integration between these sectors is one of the biggest obstacles to effective service. Healthcare professionals are often unaware of the resources available in other areas and have difficulty making the necessary referrals to ensure continuity of care. This lack of coordination results in failures in care and the exclusion of many people from health and protection services <xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B21"><sup>21</sup></xref>.</p>
					<p>Although intersectoral coordination is a relevant guideline in the SUS, it still faces significant challenges, such as the lack of a holistic view of the care process among health professionals and the scarcity of financial and human resources to implement efficient intersectoral actions. Continuing Education has the potential to overcome these barriers, empowering professionals to recognize the importance of networking and collaboration with other services. However, for this to happen, there is a need for a cultural change in healthcare institutions, which must be promoted<xref ref-type="bibr" rid="B17"><sup>17</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
					<p>Permanent Education, as a structuring policy in the SUS, is a central instrument for operationalizing Health Promotion in the daily routine of services. EPS must bring about changes in practices by problematizing the work process and integrating technical and popular knowledge<xref ref-type="bibr" rid="B29"><sup>29</sup></xref>. However, as already pointed out, there is a mismatch between what is proposed by policy and what is implemented in the territories. EPS must be committed to transforming practices, not just technical updates. It implies recognizing that health promotion is not a one-off action, but a way of organizing care and the work process <xref ref-type="bibr" rid="B30"><sup>30</sup></xref>. The construction of critical learning spaces in services can be the way to incorporate the ethical, cultural, and subjective dimensions that permeate institutional violence.</p>
				</sec>
				<sec>
					<title>Institutional Challenges and Working Conditions in the Health System</title>
					<p>The significant relationship between the increase in managers’ workloads in the Family Health Strategy (ESF) and institutional macropolitical aspects has been widely discussed. The lack of autonomy and planning significantly affects managers, generating stress, frustration, and anxiety due to the lack of control over work processes and insufficient support from the team. Due to this lack of autonomy, managers face difficulties in implementing effective changes in work processes, which negatively impacts the quality of service<xref ref-type="bibr" rid="B19"><sup>19</sup></xref>.</p>
					<p>It is common for professionals to exceed their daily workload, which results in physical and mental exhaustion. Long working hours, combined with poor working conditions, create significant physical and emotional demands, harming the well-being of healthcare professionals and, consequently, affecting the quality of care provided. Constant pressure on professionals leads to dissatisfaction, increased stress, and difficulty in maintaining humanized and effective care<xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
					<p>Factors such as low pay, dissatisfaction, and long working hours have a significant impact on the mental health of healthcare professionals. The more emotionally exhausted the worker felt, the more dissatisfied he considered himself, which could constitute institutional violence<xref ref-type="bibr" rid="B18"><sup>18</sup></xref>. These factors reveal the relationship between precarious working conditions and institutional violence, resulting in a cycle of emotional and physical exhaustion that affects the quality of care. </p>
					<p>Furthermore, the lack of appropriate working conditions contributes to a stressful scenario, making health institutions more prone to structured institutional violence. Violence manifests itself through action or omission in health institutions, with a lack of access and poor quality of services provided. Bureaucratic rules and relationships perpetuate inequalities and an asymmetry of power, contributing to precarious working conditions and poor service performance, which can lead to institutional violence<xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
					<p>The precariousness of working conditions, combined with the productivist logic imposed on health services, constitutes a significant obstacle to promoting health and overcoming institutional violence. Working relationships in the SUS are still marked by instability, overload, and low recognition, which compromise the ability of professionals to act critically and transformatively. Health Promotion, in this context, requires actions directed at users and care strategies for SUS workers, through healthy organizational environments, professional appreciation, and qualified listening<xref ref-type="bibr" rid="B31"><sup>31</sup></xref>. Investment in Continuing Education needs to be linked to these strategies, favoring the creation of spaces for listening, reflection, and change in institutional practices.</p>
				</sec>
			</sec>
			<sec sec-type="conclusions">
				<title>FINAL CONSIDERATIONS</title>
				<p>This study aimed to analyze the scientific evidence on the importance of Continuing Education in Health as a strategy for institutional violence prevention within the Health Care Network. The findings demonstrate that Continuing Education can play a central role in raising awareness among professionals, improving care quality, and developing more humane and ethical practices in health services. Institutional violence - in its multiple forms, such as neglect, discrimination, abuse of power, and mistreatment - has been identified as a significant obstacle to the effectiveness of health services, directly impacting the user experience and the morale of health professionals. </p>
				<p>Although the findings answer the guiding question of this study, it was observed that all included studies are Brazilian papers and have a level of evidence 4, which highlights a methodological limitation and the absence of studies with more robust designs, such as impact assessment studies, and the exclusion of gray literature, which may have restricted the scope of the evidence identified. The thematic categorization of data, although carefully carried out, also carries the risk of subjectivity in the analysis. Despite these limitations, the results indicate that continuing education can be a specific tool for transforming violent and dehumanizing institutional practices. Its use, when structured in an interprofessional, critical, and continuous manner, has the potential to positively impact the quality of Health Care, especially in public services where the main complaints related to institutional violence are concentrated.</p>
				<p>The findings highlight a significant gap in the production of empirical knowledge on the topic, especially regarding measuring the objective and sustainable results of EPS actions on institutional violence prevention. Investing in research with more robust qualitative and quantitative approaches can strengthen the field and provide more solid evidence for public policymaking. </p>
				<p>Therefore, future research is needed to deepen the understanding of how Continuing Education can be adapted to different contexts, such as those specific to vulnerable communities and marginalized populations, such as the LGBTQIA+ population, homeless people, and other groups historically excluded from access to healthcare. It is also essential to investigate the conditions for implementing Continuing Education, such as training programs adapted to local realities and the challenges faced by professionals in applying the knowledge acquired. Furthermore, studies that evaluate the effectiveness of Continuing Education in concretely reducing institutional violence are essential to validate interventions and propose improvements. The development of participatory methodologies, which involve users themselves in the construction of knowledge and the training of professionals, can be an important area to be explored. </p>
