				<title>RESUMO</title>
				<sec>
					<title>Objetivo:</title>
					<p><italic>Desenvolver e validar pictogramas relacionados ao uso de medicamentos, visando facilitar a compreensão e a adesão ao tratamento por usuários do Sistema Único de Saúde (SUS) de um município do Sul do Brasil.</italic></p>
				</sec>
				<sec>
					<title>Métodos:</title>
					<p><italic>Estudo descritivo, com abordagem qualitativa e quantitativa, realizado em unidade de saúde de Formigueiro-RS. Inicialmente, foram conduzidos grupos focais com profissionais de saúde e usuários do SUS para elaboração dos pictogramas. Posteriormente, cinco pictogramas foram selecionados e submetidos a teste de legibilidade com 200 participantes. Considerou-se como legível o pictograma compreendido por ≥85% dos entrevistados, conforme a norma ISO 3864-1.</italic></p>
				</sec>
				<sec>
					<title>Resultados:</title>
					<p><italic>Dos cinco pictogramas avaliados, quatro atingiram o critério de legibilidade. O pictograma “Tomar uma hora antes do café da manhã ou chimarrão” foi o menos compreendido (65%). A compreensão dos pictogramas apresentou associação estatisticamente significativa com escolaridade, ocupação e número de medicamentos de uso diário (p&lt;0,05).</italic></p>

				<sec>
					<title>Conclusão:</title>
					<p><italic>Os pictogramas se mostraram ferramentas eficazes para auxiliar a compreensão da prescrição médica, sobretudo quando associados a orientações verbais. O uso de imagens simples e culturalmente adequadas pode contribuir para o uso racional de medicamentos no SUS.</italic></p>

					<p><italic>Los pictogramas se mostraron como herramientas eficaces para apoyar la comprensión de la prescripción médica, especialmente cuando se complementan con orientaciones verbales. El uso de imágenes simples y culturalmente adecuadas puede contribuir al uso racional de medicamentos en el SUS.</italic></p>

			<title>INTRODUÇÃO</title>
			<p>Cerca de 40% da população adulta brasileira apresenta pelo menos uma doença crônica não transmissível, sendo mais frequentes as doenças cardiovasculares, neoplasias e diabetes mellitus<xref ref-type="bibr" rid="B1"><sup>1</sup></xref>. Paralelamente, observa-se o crescimento do uso de polifarmácia - definido como o uso concomitante de cinco ou mais medicamentos - com prevalência estimada de 9,4% na população geral e 18,1% entre idosos com mais de 65 anos<xref ref-type="bibr" rid="B2"><sup>2</sup></xref>. Nessa conjuntura, a combinação adequada de medicamentos pode aliviar sintomas, controlar doenças e melhorar a qualidade de vida<xref ref-type="bibr" rid="B2"><sup>2</sup></xref>. Entretanto, o uso irracional de fármacos, sua subutilização ou a baixa adesão ao tratamento podem desencadear reações adversas e interações medicamentosas<xref ref-type="bibr" rid="B2"><sup>2</sup></xref>.</p>
			<p>Entre as causas da não adesão ao plano terapêutico, destaca-se a dificuldade de compreensão da prescrição médica - relacionada à caligrafia inadequada -, informações desorganizadas e linguagem complexa<xref ref-type="bibr" rid="B3"><sup>3</sup></xref>. Elementos como nome do fármaco, posologia, indicações e efeitos adversos devem ser apresentados de forma clara para evitar o uso incorreto dos medicamentos, minimizando os agravos clínicos, pessoais e socioeconômicos decorrentes dessa prática<xref ref-type="bibr" rid="B3"><sup>3</sup></xref>. Ademais, estudos indicam associação entre baixa escolaridade e menor adesão ao tratamento farmacológico<xref ref-type="bibr" rid="B4"><sup>4</sup></xref>.</p>
			<p>Pesquisas apontam o “efeito pictórico superior”, segundo o qual há preferência cognitiva por informações transmitidas por imagens, em detrimento daquelas apresentadas de forma oral ou escrita<xref ref-type="bibr" rid="B5"><sup>5</sup></xref>. Nesse contexto, os pictogramas - símbolos gráficos padronizados - têm sido utilizados como recursos visuais complementares capazes de transmitir mensagens de maneira simples, clara e direta<xref ref-type="bibr" rid="B5"><sup>5</sup></xref>.</p>
			<p>Pictogramas apresentam-se como ferramentas promissoras para minimizar falhas na comunicação em saúde, facilitando a interpretação de informações e promovendo sua memorização<xref ref-type="bibr" rid="B6">⁶</xref>. Diante disso, este estudo teve como objetivo desenvolver e validar pictogramas relacionados ao uso de medicamentos, visando facilitar a compreensão e adesão ao tratamento por usuários do Sistema Único de Saúde (SUS) de um município do Sul do Brasil.</p>
		</sec>
		<sec sec-type="methods">
			<title>MÉTODOS</title>
			<p>Trata-se de um estudo descritivo, com abordagem qualitativa e quantitativa, desenvolvido em uma unidade de saúde (US), localizada no município de Formigueiro, Estado do Rio Grande do Sul. O município possui uma população estimada de aproximadamente 7.000 habitantes e está situado na região central do Estado, caracterizando-se por uma rede básica de saúde voltada ao atendimento da população rural e urbana. A unidade de saúde em questão realiza, em média, 2.680 atendimentos mensais.</p>
			<p>Foram incluídos participantes com idade igual ou superior a 18 anos, abrangendo profissionais da área da saúde e usuários do SUS do município. A seleção dos participantes ocorreu por amostragem consecutiva e por conveniência. Os critérios de exclusão foram: indivíduos com limitações cognitivas que impedissem a compreensão dos pictogramas; dificuldades visuais não corrigidas ou que se recusassem a assinar o Termo de Consentimento Livre e Esclarecido (TCLE). O estudo foi realizado entre os meses de outubro e novembro de 2021, em duas etapas metodológicas distintas: a primeira para elaboração dos pictogramas, e a segunda para o teste de legibilidade.</p>
			<p>Na primeira etapa, foram realizados dois grupos focais - um com profissionais da saúde (n=9) e outro com usuários do SUS (n=6). O método de grupo focal, conforme proposto por Kitzinger<xref ref-type="bibr" rid="B7"><sup>7</sup></xref>, caracteriza-se por discussões em grupo que visam explorar, por meio da interação entre os participantes, percepções sobre um tema específico. Durante os encontros, foram utilizadas perguntas disparadoras, como: “Se você tivesse que criar um desenho que transmitisse a informação (...), esse desenho seria constituído de...”, variando a informação conforme cinco situações previamente estabelecidas: “tomar o medicamento 1 hora antes do café da manhã ou chimarrão”, “tomar junto com o almoço”, “tomar de manhã ao acordar”, “tomar à noite” e “relógio marcando 4 horas”. Essas situações foram definidas a partir das orientações de uso mais recorrentes nos rótulos e prescrições observadas na unidade de saúde.</p>
			<p>As respostas dos participantes foram registradas em áudio, posteriormente transcritas e analisadas de acordo com as etapas da Análise de Conteúdo propostas por Bardin<xref ref-type="bibr" rid="B8"><sup>8</sup></xref>: pré-análise, exploração do material e tratamento dos resultados. As sugestões com maior frequência e grau de concordância serviram de base para o desenvolvimento dos pictogramas.</p>
			<p>A criação dos pictogramas foi realizada com o apoio técnico da Coordenadoria de Tecnologia Educacional (CTE) da Universidade Federal de Santa Maria (UFSM). O CTE, vinculado à Pró-Reitoria de Graduação, atua no desenvolvimento de recursos educacionais e tecnológicos, colaborando com projetos voltados ao ensino-aprendizagem em diferentes modalidades.</p>
			<p>Na segunda etapa do estudo, foi conduzido o teste de legibilidade dos pictogramas. Para essa análise, utilizou-se como referência a norma ISO 3864-19, que considera legível um pictograma que atinge, no mínimo, 85% de acertos na interpretação pelos usuários.</p>
			<p>O cálculo amostral foi baseado na população de usuários que retiram medicamentos mensalmente na Farmácia Municipal, estimada em 2.680 indivíduos. Adotando-se prevalência esperada de 85%, margem de erro de 5%, nível de confiança de 95% e acréscimo de 10% para possíveis perdas, a amostra foi estimada em 200 participantes. O cálculo foi realizado por meio do <italic>software</italic> EpiInfo® versão 6.0.</p>
			<p>A coleta de dados foi realizada em horários alternados, nos meses de outubro e novembro de 2021. Os participantes responderam a um questionário composto por duas seções: a primeira com variáveis sociodemográficas; e a segunda contendo os pictogramas desenvolvidos. A compreensão de cada pictograma foi avaliada por meio da pergunta: “Se você tivesse que tomar um medicamento e a informação de uso fosse representada por esta figura, o que você entenderia?”.</p>
			<p>Os dados foram organizados em banco eletrônico no Microsoft Excel®, com posterior análise estatística no <italic>software</italic> SPSS Statistics®, versão 19. As variáveis foram expressas em frequências absolutas e relativas. Para análise de associação entre variáveis, foi aplicado o teste Qui-quadrado, adotando-se nível de significância de 5% e intervalo de confiança de 95%.</p>
			<p>Para análise da faixa etária, utilizou-se a classificação da Organização Mundial da Saúde (OMS), que categoriza os adultos em: adultos jovens (15-30 anos), adultos em idade madura (31-45 anos), adultos em idade de transição (46-60 anos) e idosos (acima de 60 anos)<xref ref-type="bibr" rid="B10"><sup>10</sup></xref>.</p>
			<p>O estudo foi conduzido conforme os preceitos éticos para pesquisas com seres humanos, seguindo a Resolução nº 466, de 12 de dezembro de 2012, do Conselho Nacional de Saúde. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Santa Maria (UFSM), sob o parecer nº 4.267.152 e CAAE nº 20060719.6.0000.5346. Todos os participantes assinaram o TCLE antes do início das entrevistas.</p>
		</sec>
		<sec sec-type="results">
			<title>RESULTADOS</title>
			<p>O grupo focal realizado com profissionais da saúde foi constituído por seis mulheres e três homens, sendo quatro enfermeiros, dois médicos, uma fisioterapeuta, uma odontóloga e uma farmacêutica. O tempo de experiência profissional na instituição variou entre um e dez anos, predominando o trabalho em turno diurno. Embora o estudo tenha sido desenvolvido em uma unidade básica de saúde (UBS), alguns dos participantes do grupo de profissionais atuavam também em outros serviços do município, evidenciando vínculos empregatícios múltiplos com a rede municipal de saúde.</p>
			<p>Quanto às unidades de atendimento, dois profissionais atuavam no pronto-socorro adulto e na internação clínica, seis estavam vinculados à unidade básica de saúde e um trabalhava na Farmácia Municipal. O grupo de usuários do serviço de saúde, por sua vez, foi composto por seis mulheres e dois homens.</p>
			<p>Após a transcrição das respostas dos grupos focais, utilizou-se o grau de concordância entre os participantes como critério para definir a constituição ideal dos pictogramas. As expressões mais frequentes e as sugestões coincidentes entre os sujeitos foram sistematizadas e orientaram a elaboração inicial dos pictogramas (<xref ref-type="table" rid="t1">Tabela I</xref>).</p>

			<p>A partir da sistematização dos dados dos grupos focais, 13 pictogramas foram criados. Dentre esses, cinco foram selecionados pelos pesquisadores para a etapa de validação (<xref ref-type="fig" rid="f1">Figura 1</xref>), com base na relevância e recorrência das informações transmitidas.</p>
			<p>Na pesquisa de legibilidade, a amostra foi composta majoritariamente por mulheres (65%), sendo a faixa etária predominante a de idade madura (33,8%). Em relação à escolaridade, 40,5% possuíam ensino médio completo, e a maior parte dos entrevistados apresentava renda entre um e três salários mínimos (50%). Destacou-se também a presença de profissionais do Grupo 2 da Classificação Brasileira de Ocupações (24,1%), sendo que 56% faziam uso de um a três medicamentos por dia (<xref ref-type="table" rid="t2">Tabela II</xref>).</p>

			<p>O pictograma com maior índice de acerto foi o número 4 (“Tomar à noite”), compreendido por 97,5% dos participantes. Em seguida, destacaram-se os pictogramas número 2 (“Tomar junto com o almoço” - 95,5%), número 5 (“Relógio marcando 4 horas” - 92,5%), e número 3 (“Tomar ao acordar” - 90,5%). O Pictograma 1 (“Tomar 1 hora antes do café da manhã ou chimarrão”) apresentou menor índice de acerto (65%), não sendo considerado legível segundo os critérios da ISO 3864-1 (<xref ref-type="table" rid="t3">Tabela III</xref>).</p>
			<p>As principais dificuldades relacionadas ao Pictograma 1 referem-se à interpretação do horário e do café da manhã, além de respostas vagas ou ausência de entendimento por parte dos participantes. Os equívocos mais comuns envolveram confusão com o uso matinal ou desconhecimento do termo “chimarrão”.</p>

			<p>Na análise de associação entre o número de acertos e variáveis socioeconômicas (<xref ref-type="table" rid="t4">Tabela IV</xref>), observou-se significância estatística (p&lt;0,05) para as variáveis escolaridade, ocupação e número de medicamentos de uso diário.</p>

			<title>DISCUSSÃO</title>
			<p>Este estudo teve como objetivo propor a criação e a validação de pictogramas para favorecer a compreensão da prescrição médica por usuários do Sistema Único de Saúde (SUS), considerando as especificidades culturais e sociodemográficas locais. Um aspecto cultural importante identificado durante os grupos focais foi o hábito regional do consumo de chimarrão, típico da região Sul do Brasil. Tal elemento foi incorporado em um dos pictogramas, visando maior identificação dos usuários com as mensagens transmitidas visualmente.</p>
			<p>Observou-se que os usuários do SUS sugeriram mais recursos gráficos e numéricos do que os profissionais da saúde. Essa diferença pode estar relacionada ao nível de escolaridade e ao conhecimento técnico dos grupos, influenciando a forma como interpretam e constroem as informações visuais<xref ref-type="bibr" rid="B11"><sup>11</sup></xref>. Ressalta-se, portanto, a importância de validar materiais educativos com o público-alvo, garantindo sua adequação linguística e comunicacional<xref ref-type="bibr" rid="B12"><sup>12</sup></xref>.</p>
			<p>Pictogramas, ao recorrerem a símbolos do cotidiano, promovem a associação de conceitos médicos a imagens familiares, facilitando a decodificação de informações, especialmente em contextos de baixa literacia em saúde<xref ref-type="bibr" rid="B13"><sup>13</sup></xref>. A representação visual de situações conhecidas pelo usuário potencializa a eficácia das orientações, convertendo mensagens técnicas em instruções claras e compreensíveis.</p>
			<p>A prevalência do sexo feminino entre os participantes da pesquisa de legibilidade reflete um padrão consolidado na literatura: mulheres tendem a procurar com mais frequência os serviços de saúde, tanto para cuidados próprios quanto no acompanhamento de familiares, especialmente crianças e idosos<xref ref-type="bibr" rid="B1"><sup>1</sup></xref>. Essa maior presença feminina está associada a uma percepção mais aguçada dos sintomas e maior responsabilidade no cuidado de si e do outro.</p>
			<p>No que tange à idade, observou-se predominância de indivíduos em idade madura. Tal faixa etária coincide com maior propensão a condições crônicas e, portanto, maior uso de medicamentos. Esse comportamento pode ser interpretado também como uma busca por autonomia frente ao próprio cuidado, algo que se intensifica com o envelhecimento e com o aumento da longevidade<xref ref-type="bibr" rid="B14"><sup>14</sup></xref>. Além disso, adultos jovens e mulheres, em especial, também acessam os serviços em razão de demandas relacionadas à saúde reprodutiva e aos cuidados com os filhos<xref ref-type="bibr" rid="B15"><sup>15</sup></xref>.</p>
			<p>Embora a amostra tenha apresentado elevada escolaridade, deve-se considerar que muitos usuários do SUS hoje possuem plano de saúde privado, mas continuam utilizando serviços públicos por questões de acessibilidade ou cobertura limitada em planos<xref ref-type="bibr" rid="B16"><sup>16</sup></xref>. Segundo a Agência Nacional de Saúde Suplementar, essa tendência ocorre mesmo entre indivíduos com maior escolaridade<xref ref-type="bibr" rid="B17"><sup>17</sup></xref>, o que reforça a relevância de estratégias de comunicação universal, como os pictogramas.</p>
			<p>Em contrapartida, foi evidenciada também a predominância de ocupações comumente associadas a menor escolaridade formal, como diaristas, cozinheiras, vigilantes, porteiros e aposentados. Tal achado pode sugerir avanços no acesso à educação formal, mesmo em contextos socioeconômicos menos favorecidos - o que não elimina a necessidade de materiais acessíveis a todos os níveis de alfabetização.</p>
			<p>O uso de múltiplos medicamentos entre os participantes está de acordo com a elevada prevalência de doenças crônicas, como hipertensão e diabetes. O aumento no número de fármacos eleva o risco de automedicação, erros posológicos e armazenagem inadequada dos medicamentos, comprometendo sua eficácia e segurança<xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
			<p>Em relação à legibilidade, apenas um pictograma não alcançou os critérios da norma ISO 3864-1<xref ref-type="bibr" rid="B9"><sup>9</sup></xref>. O Pictograma 1 concentrou o maior número de erros, provavelmente devido à complexidade de transmitir múltiplas informações em uma única imagem. Estudos anteriores defendem que pictogramas eficazes devem ser simples, conter um único conceito visual e ser facilmente interpretados<xref ref-type="bibr" rid="B21"><sup>21</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>.</p>
			<p>De modo geral, a compreensão dos pictogramas mostrou associação significativa com o grau de escolaridade, ocupação e número de medicamentos utilizados. Cruzeta<xref ref-type="bibr" rid="B23"><sup>23</sup></xref> aponta que pessoas com maior escolaridade interpretam melhor as prescrições médicas. Contudo, a análise deste estudo indica que, mesmo em ocupações que exigem menor instrução formal, a compreensão dos pictogramas foi elevada, demonstrando sua aplicabilidade ampla<xref ref-type="bibr" rid="B26"><sup>26</sup></xref>.</p>
			<p>Além disso, a associação de pictogramas à orientação verbal mostrou-se uma estratégia promissora para aumentar a adesão medicamentosa, com potencial para otimizar o uso racional de medicamentos, reduzir erros e contribuir para a sustentabilidade dos sistemas de saúde pública<xref ref-type="bibr" rid="B5"><sup>5</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B13"><sup>13</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B26"><sup>26</sup></xref>.</p>
		</sec>
		<sec sec-type="conclusions">
			<title>CONCLUSÃO</title>
			<p>A metodologia adotada por meio de grupos focais e entrevistas demonstrou-se eficaz para o desenvolvimento de pictogramas compreensíveis e culturalmente adequados. Dos cinco pictogramas testados, quatro atingiram critérios de legibilidade segundo a ISO 3864-1, e a compreensão esteve significativamente associada à escolaridade, ocupação e ao número de medicamentos de uso diário.</p>
			<p>A inserção de pictogramas como complemento à orientação verbal pode favorecer a adesão ao tratamento e a compreensão da prescrição, reforçando o uso racional de medicamentos. Contudo, recomenda-se a realização de novos estudos, com amostras mais amplas e diversas, para aprimorar e adaptar as figuras às demandas reais da população brasileira.</p>

				<title>ABSTRACT</title>
				<sec>
					<title>Objective:</title>
					<p> To develop and validate pictograms related to medication use in order to improve understanding and treatment adherence among users of the Brazilian Unified Health System (SUS) in a municipality in Southern Brazil. </p>
				</sec>
				<sec>
					<title>Methods:</title>
					<p> This was a descriptive study with both qualitative and quantitative approaches, conducted at a health unit in Formigueiro, Rio Grande do Sul, Brazil. Initially, focus groups were held with health professionals and SUS users to design the pictograms. Subsequently, 5 pictograms were selected and submitted to a legibility test involving 200 participants. A pictogram was considered legible if it was understood by ≥ 85% of respondents, in accordance with ISO 3864-1. </p>
				</sec>
				<sec>
					<title>Results:</title>
					<p> Of the 5 pictograms evaluated, 4 met the legibility criterion. The pictogram “Take 1 hour before breakfast or chimarrão” was the least understood (65%). Pictogram comprehension showed a statistically significant association with education level, occupation, and the number of daily medications used (p&lt;0.05). </p>
				</sec>
				<sec>
					<title>Conclusion:</title>
					<p> Pictograms proved to be effective tools in aiding comprehension of medical prescriptions, especially when combined with verbal instructions. The use of simple, culturally appropriate images may support the rational use of medications within the SUS.</p>

				<title>INTRODUCTION</title>
				<p>Approximately 40% of the adult Brazilian population has at least 1 noncommunicable chronic disease, with cardiovascular conditions, cancer, and diabetes mellitus being the most prevalent<xref ref-type="bibr" rid="B1"><sup>1</sup></xref>. At the same time, the use of polypharmacy - defined as the concurrent use of 5 or more medications - is on the rise, with an estimated prevalence of 9.4% in the general population and 18.1% among individuals over 65 years of age<xref ref-type="bibr" rid="B2"><sup>2</sup></xref>. The appropriate combination of medications can relieve symptoms, control diseases, and improve quality of life<xref ref-type="bibr" rid="B2"><sup>2</sup></xref>. However, irrational drug use, underuse, or poor treatment adherence may lead to adverse reactions and drug interactions<xref ref-type="bibr" rid="B2"><sup>2</sup></xref>.</p>
				<p>One of the main causes of non-adherence to treatment is the difficulty in understanding medical prescriptions, often due to illegible handwriting, disorganized information, or complex language<xref ref-type="bibr" rid="B3"><sup>3</sup></xref>. Essential elements such as drug name, dosage, indications, and side effects must be presented clearly to prevent medication errors and minimize clinical, personal, and socioeconomic harms<xref ref-type="bibr" rid="B3"><sup>3</sup></xref>. Furthermore, studies have shown a link between low educational attainment and poor adherence to pharmacological treatment<xref ref-type="bibr" rid="B4"><sup>4</sup></xref>.</p>
				<p>Research highlights the “picture superiority effect,” which refers to a cognitive preference for information conveyed through images rather than oral or written formats<xref ref-type="bibr" rid="B5"><sup>5</sup></xref>. Pictograms - i.e., standardized graphic symbols - have been increasingly used as visual aids capable of conveying messages in a simple, clear, and direct manner<xref ref-type="bibr" rid="B5"><sup>5</sup></xref>.</p>
				<p>Pictograms have emerged as promising tools for minimizing communication failures in health care, facilitating the interpretation and retention of information<xref ref-type="bibr" rid="B6"><sup>6</sup></xref>. Therefore, the present study aimed to develop and validate pictograms related to medication use, with the goal of improving comprehension and treatment adherence among users of the Brazilian Unified Health System (SUS) in a city in Southern Brazil.</p>
			</sec>
			<sec sec-type="methods">
				<title>METHODS</title>
				<p>This was a descriptive study with both qualitative and quantitative approaches, conducted at a health unit located in the city of Formigueiro, state of Rio Grande do Sul, Brazil. Formigueiro has an estimated population of approximately 7,000 inhabitants and is located in the central region of the state, featuring a basic health care network serving both rural and urban populations. The health unit in question provides an average of 2,680 consultations per month.</p>
				<p>Participants included individuals aged 18 years or older, comprising health professionals and users of the SUS in the city. Participants were selected through consecutive and convenience sampling. Exclusion criteria included individuals with cognitive impairments that prevented them from understanding the pictograms, uncorrected visual impairments, or those who declined to sign the informed consent form. The study was conducted between October and November 2021 and involved 2 distinct methodological stages: the first focused on the development of pictograms, and the second on testing their legibility.</p>
				<p>In the first stage, 2 focus groups were conducted - one with health professionals (n = 9) and another with SUS users (n = 6). The focus group method, as proposed by Kitzinger<xref ref-type="bibr" rid="B7"><sup>7</sup></xref>, involves group discussions aimed at exploring perceptions on a specific topic through participant interaction. During the sessions, participants were asked prompting questions such as: “If you had to create a drawing that conveyed the information (…), what would it look like?” The question varied according to 5 predefined situations: “take the medication 1 hour before breakfast or chimarrão,” “take with lunch,” “take in the morning upon waking,” “take at night,” and “clock showing 4 o’clock.” These scenarios were based on the most commonly observed instructions in prescriptions and medication labels at the health unit.</p>
				<p>Participant responses were audio-recorded, transcribed, and analyzed using the steps of Content Analysis proposed by Bardin<xref ref-type="bibr" rid="B8"><sup>8</sup></xref>: pre-analysis, material exploration, and treatment of results. The most frequent and agreed-upon suggestions served as the basis for pictogram development.</p>
				<p>The pictograms were created with technical support from the Center for Educational Technology (CTE) at the Universidade Federal de Santa Maria (UFSM). The CTE, part of the Office of Undergraduate Studies, specializes in the development of educational and technological resources and collaborates on teaching and learning projects across various modalities.</p>
				<p>In the second stage, the pictograms underwent legibility testing. For this analysis, ISO 3864-1<xref ref-type="bibr" rid="B9"><sup>9</sup></xref> was used as a reference, which considers a pictogram legible when at least 85% of users correctly interpret its meaning.</p>
				<p>Sample size was calculated based on the estimated number of users who collect medications monthly at the Municipal Pharmacy (2,680 individuals). Assuming an expected prevalence of 85%, a 5% margin of error, 95% confidence level, and a 10% increase to account for potential losses, the required sample was estimated at 200 participants. Calculation was performed using EpiInfo<sup>®</sup> version 6.0.</p>
				<p>Data collection took place at various times of day during October and November 2021. Participants completed a questionnaire consisting of 2 sections: the first covered sociodemographic variables, and the second presented the developed pictograms. Comprehension of each pictogram was assessed using the question: “If you had to take a medication and this figure represented the instructions, what would you understand it to mean?”</p>
				<p>Data were organized in an electronic database using Microsoft Excel<sup>®</sup> and analyzed statistically with IBM SPSS Statistics for Windows, version 19 (IBM Corp., Armonk, N.Y., USA). Variables were expressed in absolute and relative frequencies. Associations between variables were analyzed using the chi-square test, adopting a 5% significance level and a 95% confidence interval.</p>
				<p>Age groups were classified according to the World Health Organization (WHO), which categorizes adults as follows: young adults (15-30 years), middle-aged adults (31-45 years), late middle-aged adults (46-60 years), and older adults (over 60 years) <sup>(</sup><xref ref-type="bibr" rid="B10"><sup>10</sup></xref>.</p>
				<p>The study followed the ethical principles outlined in Resolution No. 466 of December 12, 2012, of the Brazilian National Health Council. The research project was approved by the Research Ethics Committee at UFSM, under approval number 4.267.152 and CAAE number 20060719.6.0000.5346. All participants signed the informed consent form prior to interviews.</p>
			</sec>
			<sec sec-type="results">
				<title>RESULTS</title>
				<p>The focus group with health professionals included 6 women and 3 men: 4 nurses, 2 physicians, 1 physical therapist, 1 dentist, and 1 pharmacist. Their professional experience at the institution ranged from 1 to 10 years, with most working daytime shifts. Although the study was conducted at a primary health care unit, some of the professionals also worked in other municipal health services, indicating multiple employment ties within the local health care network.</p>
				<p>Regarding service allocation, 2 professionals worked in the adult emergency room and clinical ward, 6 were affiliated with the primary health care unit, and 1 worked at the Municipal Pharmacy. The group of SUS users consisted of 6 women and 2 men.</p>
				<p>After transcription of the focus group responses, the degree of agreement among participants was used as the primary criterion for defining the ideal composition of pictograms. The most frequently mentioned expressions and overlapping suggestions were systematized and guided the initial design of the pictograms (<xref ref-type="table" rid="t5">Table I</xref>).</p>

				<p>Based on the systematization of data from the focus groups, 13 pictograms were created. Of these, 5 were selected by the researchers for the validation phase (<xref ref-type="fig" rid="f2">Figure 1</xref>), based on the relevance and recurrence of the information conveyed. In the legibility survey, the sample consisted predominantly of women (65%), with the most represented age group being middle-aged adults (33.8%). Regarding education level, 40.5% had completed high school, and most participants reported a monthly income of 1-3 minimum wages (50%). Notably, 24.1% were professionals classified under Group 2 of the Brazilian Classification of Occupations, and 56% reported using 1-3 medications per day (<xref ref-type="table" rid="t6">Table II</xref>).</p>

				<p>The pictogram with the highest accuracy rate was number 4 (“Take at night”), correctly interpreted by 97.5% of participants. It was followed by pictogram number 2 (“Take with lunch” - 95.5%), number 5 (“Clock showing 4 o’clock” - 92.5%), and number 3 (“Take upon waking” - 90.5%). Pictogram 1 (“Take 1 hour before breakfast or chimarrão”) had the lowest accuracy rate (65%) and was not considered legible according to ISO 3864-1 criteria (<xref ref-type="table" rid="t7">Table III</xref>).</p>
				<p>The main difficulties related to Pictogram 1 involved interpreting the timing and identifying the breakfast reference, along with vague answers or a lack of understanding from participants. The most common errors included confusion with morning medication use and unfamiliarity with the term chimarrão.</p>

				<p>In the analysis of the association between the number of correct responses and socioeconomic variables (<xref ref-type="table" rid="t8">Table IV</xref>), statistically significant results (p &lt; 0.05) were observed for the variables education level, occupation, and number of daily medications used.</p>

				<title>DISCUSSION</title>
				<p>This study aimed to propose the creation and validation of pictograms to improve the understanding of medical prescriptions by users of the SUS, considering local cultural and sociodemographic characteristics. One important cultural aspect identified during the focus groups was the regional habit of consuming chimarrão, which is typical of Southern Brazil. This element was incorporated into 1 of the pictograms to enhance user identification with the visual messages.</p>
				<p>SUS users suggested more graphic and numerical elements than health professionals. This difference may be related to the groups’ educational levels and technical knowledge, which influence how they interpret and construct visual information<xref ref-type="bibr" rid="B11"><sup>11</sup></xref>. This highlights the importance of validating educational materials with the target audience to ensure linguistic and communicative appropriateness<xref ref-type="bibr" rid="B12"><sup>12</sup></xref>.</p>
				<p>By relying on familiar symbols, pictograms promote the association of medical concepts with everyday images, facilitating information decoding - especially in contexts of low health literacy<xref ref-type="bibr" rid="B13"><sup>13</sup></xref>. The visual representation of familiar situations enhances the effectiveness of instructions, transforming technical messages into clear and understandable guidance.</p>
				<p>The predominance of female participants in the legibility study reflects a well-established pattern in the literature: women are more likely to seek health care services, both for their own needs and when caring for family members, especially children and older adults<xref ref-type="bibr" rid="B1"><sup>1</sup></xref>. This higher female participation is associated with greater symptom awareness and a stronger sense of responsibility for self-care and caregiving.</p>
				<p>Regarding age, most participants were middle-aged adults, a group more likely to experience chronic conditions and, therefore, higher medication use. This behavior may also reflect a pursuit of autonomy in health management, which tends to increase with aging and longer life expectancy<xref ref-type="bibr" rid="B14"><sup>14</sup></xref>. Additionally, young adults and women often access services for reproductive health or childcare-related needs<xref ref-type="bibr" rid="B15"><sup>15</sup></xref>.</p>
				<p>Although the sample had a relatively high level of education, it is important to consider that many SUS users also have private health insurance but continue to use public services due to accessibility or coverage limitations<xref ref-type="bibr" rid="B16"><sup>16</sup></xref>. According to the National Supplementary Health Agency, this trend is seen even among individuals with higher education<xref ref-type="bibr" rid="B17"><sup>17</sup></xref>, reinforcing the need for universal communication strategies such as pictograms.</p>
				<p>On the other hand, the sample also showed a predominance of occupations typically associated with lower formal education, such as domestic workers, cooks, security guards, janitors, and retirees. This may suggest progress in access to formal education even among socioeconomically disadvantaged groups - though it does not eliminate the need for materials that are accessible to all literacy levels.</p>
				<p>The frequent use of multiple medications among participants aligns with the high prevalence of chronic diseases such as hypertension and diabetes. As the number of medications increases, so does the risk of self-medication, dosing errors, and improper storage, which can compromise the effectiveness and safety of treatment<xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>), (</sup><xref ref-type="bibr" rid="B20"><sup>20</sup></xref>.</p>
				<p>In terms of legibility, only 1 pictogram failed to meet ISO 3864-1 standards.<xref ref-type="bibr" rid="B9"><sup>9</sup></xref> Pictogram 1 had the highest number of errors, likely due to the challenge of conveying multiple pieces of information in a single image. Previous studies have emphasized that effective pictograms should be simple, convey only 1 visual concept, and be easily interpreted<xref ref-type="bibr" rid="B21"><sup>21</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref>.</p>
				<p>Overall, pictogram comprehension was significantly associated with education level, occupation, and number of medications used. Cruzeta et al.<xref ref-type="bibr" rid="B23"><sup>23</sup></xref> noted that individuals with higher education levels tend to interpret medical prescriptions more accurately. However, the analysis in this study indicates that even among occupations requiring less formal education, pictogram comprehension was high - demonstrating their broad applicability<xref ref-type="bibr" rid="B26"><sup>26</sup></xref>.</p>
				<p>Furthermore, combining pictograms with verbal instructions proved to be a promising strategy to increase medication adherence, with the potential to promote rational medicine use, reduce errors, and support the sustainability of public health care systems<xref ref-type="bibr" rid="B5"><sup>5</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B13"><sup>13</sup></xref><sup>),(</sup><xref ref-type="bibr" rid="B26"><sup>26</sup></xref>.</p>

			<sec sec-type="conclusions">
				<title>CONCLUSION</title>
				<p>The methodology adopted through focus groups and interviews proved effective for the development of culturally appropriate and easily understood pictograms. Of the 5 pictograms tested, 4 met the legibility criteria established by ISO 3864-1, and comprehension was significantly associated with education level, occupation, and the number of medications used daily.</p>
				<p>The incorporation of pictograms as a complement to verbal guidance may enhance treatment adherence and prescription understanding, thereby reinforcing the rational use of medications.
